STEP 7: a shorter semaglutide trial in a predominantly East Asian population

The 44-week endpoint and inclusion of people with and without diabetes distinguish this report from other STEP studies.

Start with place, population and time

STEP 7 randomized 375 adults with overweight or obesity, with or without type 2 diabetes, at sites in China, Hong Kong, Brazil and South Korea. It compared semaglutide 2.4 mg weekly with placebo plus diet and activity intervention. The primary endpoint was assessed at 44 weeks. [1]

The phrase predominantly East Asian describes the study population more accurately than assuming every participant had the same background. The study geography also includes more than one health system. A reader comparing this with another STEP trial should begin with these actual features rather than its trial number.

The result at week 44

Average weight change was −12.1% with semaglutide and −3.6% with placebo. The reported proportion achieving at least 5% loss was 85% versus 31% among participants with the week-44 assessment. Gastrointestinal disorders occurred in 67% and 36% of the treated safety groups. Novo Nordisk funded the trial. [1]

An average change and a threshold proportion answer different questions. The first summarizes the magnitude of change across a group; the second counts how many reached a particular cutoff. Neither should be read as the probability that a new individual will attain a promised result by a fixed date.

Why 44 weeks cannot become 68 weeks

A shorter observation period changes the question. It is inappropriate to scale the result up linearly to predict a later endpoint, or to label the study inferior because a longer trial reports a larger percentage. Weight trajectories are not established by multiplying one endpoint by the ratio of study durations.

The inclusion of participants with and without diabetes also matters when comparing averages. That mix is part of the trial, not a variable a consumer can remove by intuition. A meaningful comparison with STEP 6 needs attention to eligibility and design as well as geography.

The distinction between enrollment and measurement is worth retaining in threshold outcomes. If a percentage is based on participants assessed at the endpoint, report that basis. It should not silently become a count out of everyone originally randomized without checking the analysis.

A disciplined way to use the evidence

Write the result as a sentence containing the population, comparator and time point before bringing it to an appointment. This simple habit prevents a number from becoming detached from the circumstances that produced it. Ask the clinician which parts of the evidence are most relevant to your own diagnosis and goals.

For comparing a service, check the actual follow-up arrangement separately. A research citation does not establish how frequently a clinic reviews progress or who answers questions. Our ongoing-support guide helps connect that practical discussion with the time horizon you and the clinician are considering.
